The UNAIDS 95-95-95 Target
To combat HIV/AIDS effectively, UNAIDS has set the 95-95-95 target, aiming for 95% of people living with HIV to know their status, 95% of those diagnosed to receive antiretroviral therapy, and 95% of those on treatment to achieve viral suppression by 2030. Achieving these targets is crucial for reducing new HIV infections and improving health outcomes worldwide. HIV testing is the first step in meeting these goals, ensuring that more individuals receive timely diagnosis and treatment.
Malaysia’s Progress on the 95-95-95 Target
According to the 2024 Global AIDS Monitoring Country Progress Report for Malaysia, by the end of 2023:
- 84% of people living with HIV (PLHIV) were diagnosed and knew their status.
- 68% of those diagnosed were receiving treatment, indicating a gap in treatment and care.
- 89% of those on treatment achieved viral suppression.
These statistics highlight the importance of increasing HIV testing and improving treatment accessibility to reach the 95-95-95 targets.
Breaking the Stigma Around HIV Testing
Stigma remains a major barrier to HIV testing. Dr. Khairil Erwan Khalid, an Infectious Disease Physician at Hospital Kuala Lumpur, explains, “The main reason would be stigma and fear of discrimination. Many people worry about being judged by the community, family members especially, and even health care providers. Other possible reasons would be lack of awareness and misinformation, and also confidentiality concerns.” Addressing these concerns through education, supportive healthcare environments, and promoting confidential testing options is vital in encouraging more people to get tested.
The Role of Anonymity in HIV Testing
Confidentiality is a key factor in encouraging people to take an HIV test. Mr. Chung Han Yang, Deputy Executive Director, Malaysian AIDS Council, emphasizes, “It is a very important element of the HIV testing services across all the different service points (HIV Self Testing, HIV test at GPs, Public Clinics, Community based testing by NGO health workers or NGO clinics) to ensure individuals feeling comfortable and safe to come forward for HIV test to know their status, especially for highly stigmatized populations, young people and vulnerable populations.” Many individuals fear the social consequences of an HIV-positive diagnosis, which prevents them from seeking testing. Clinics and healthcare providers must ensure that testing is discreet and private to create a safer environment for individuals to know their status.
Expanding HIV Testing Accessibility
Despite its importance, HIV testing is not always easily accessible. While rapid HIV tests are available in hospitals and specialized clinics, they are less common in general practitioner (GP) clinics. Dr. Freddy, a General Practitioner at a GP clinic in Klang Valley, explains, “There isn’t enough demand for HIV tests.” This highlights the need for increased awareness and education on the benefits of HIV testing. Encouraging routine HIV screening as part of general health check-ups could help normalize testing and improve accessibility.
