Short Frenulum or Frenulum Breve

“Doctor. I have been bleeding after having an intimate relationship. I feel tight and it always bleeds at the same spot every time.”

Every now and then, I will have a man come to see me for this specific complaint. Some may have issues with tight foreskin, STD which manifested as skin lesions or ulcers or even too hard erection. More often than not, some of them suffer from short frenulum. 

The frenulum is a band of tissue that connects the penile head to the foreskin. It usually becomes longer as we age; however, in some men, it does not. This may happen due to a very tight foreskin, or sometimes even a long and redundant foreskin may reduce the chance of it becoming longer. 

When men are younger, they often don’t notice issues with the frenulum, as it typically doesn’t cause any problems. However, once they grow up and become sexually active, they may begin to experience discomfort. A short or tight frenulum, known as frenulum breve, can lead to pain or tearing during intercourse. Pain happens due to overstretching of the tissue, which can lead to tearing of the frenulum. Tears often heal with medication and wound care. 

However, frequent tears in the frenulum can cause it to become less elastic over time, which may result in even more frequent tearing and discomfort. This cycle can worsen if not addressed, as the tissue may heal with less flexibility, making future injuries more likely.

A visit to an experienced doctor is advisable as a skilled and trained doctor is necessary in treating this condition, especially in the more severe or recurrent cases. 

Treatment options for a short frenulum may include:

  1. Topical treatments: A doctor may recommend using anti-inflammatory creams to help reduce inflammation and improve the skin’s elasticity.
  2. Stretching exercises: Gentle stretching of the frenulum, done under the guidance of a doctor, can sometimes help increase its length and flexibility, however, this is not guaranteed and may require a long time to achieve even the 
  3. Frenuloplasty: In more severe cases, a minor surgical procedure called frenuloplasty may be performed to lengthen the frenulum and prevent further tearing.
